Welcome Remarks - Executive Director Acknowledge key attendees (Board members, community partners, funders, etc.)

Overview of BH Program and New Location - Behavioral Health Practice Manager and Clinical Director Behavioral Health Program Overview (10 minutes) Introduction to Behavioral Health services offered Explanation of care model (integrated care, same-day access, etc.) Overview of staffing (providers, therapists, support staff). Expected community impact and goals for the new location 

3. Brief overview of the open house 

4. Chamber Certificate Presentation 5. Cut Ribbon & Take Pictures 

4:25: Open House Tours – Information Stations until 6:00 pm

Ribbon cuttings and groundbreakings are held at member investor facilities that have celebrated one of the following successes:

• Broke ground or opened a new facility
• Moved to a new location
• Remodeled an existing facility
• Welcomed new ownership
• Underwent a significant re-branding or name change
