Lakeland Care, Inc. is a non-profit, Managed Care Organization (MCO) supporting over 7,000 members across 22 counties and two Tribes in Northeast and North Central Wisconsin. We deliver world-class service by providing long-term care supports to eligible frail elders and individuals with physical and intellectual/developmental disabilities through Wisconsin's Family Care program. Our goal is to help our members live as independently as possible because we know how important it is to remain connected to family, friends and community.
